29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas: On or about April 5, 2013, at the worksite located at 3801 West 8th Street in Little Rock, Arkansas, front of building being constructed, wooden boards with protruding nails were scattered on the ground. This condition exposed employees to puncture hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about April 5, 2013, at the worksite located at 3801 West 8th Street in Little Rock, Arkansas, front of building being constructed, employees working from the approximately 6 ft., 5" extended upper decked hallway area installing Typar Metro Wrap to wall framing, were not protected from falling to lower ground level. Employees were exposed to an approximately 9 ft. fall from elevation.
