29 CFR 1926.403(e): Conductors were not spliced or joined with splicing devices designed for the use or by brazing, welding, or soldering with a fusible metal or alloy. All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device designed for the purpose.        a) Stairwell B:        Employees used temporary lighting and outlet devices installed on a wood pole scaffold for working with electric powered hand tools. The lighting fixtures and outlet devices were wired to a circuit breaker through temporary electrical conductors, which were not spliced or joined with splicing devices designed for the use. All splices and joints were not covered with an insulation equivalent to that of the conductors or with an insulating device designed for the purpose. On or about 04/05/13. 29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.       a) Stairwell B:       Employees used electric powered hand tools to remove plaster and stucco on the stairwell walls. The hand tools were connected to extension cords, which were routed to temporary outlet devices installed on a wood pole scaffold. The hand tools and extension cords were not provided with ground fault circuit interrupters. On or about 04/05/13. 29 CFR 1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person.       a) Stairwell B:       Two employees constructed a wood pole scaffold on the staircase around the stairwell from basement level to roof bulkhead and worked from this scaffold. The scaffold was not erected by experienced and trained employees under the supervision and direction of a competent person. Employees who erected and worked on the scaffolds were exposed to hazards, including but not limited to: Diagonal bracing were not installed across vertical pole supports, and wooden scaffold poles were improperly spliced. On or about 04/05/13.       NOTE:  IN ADDITION TO CERTIFICATION OF ABATEMENT, THE EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION OF ABATEMENT FOR THIS ITEM. Employees could not recognize hazards associated with pole scaffolds, and were exposed to hazards, including but not limited to: Diagonal bracing were not installed across vertical pole supports, and wooden scaffold poles were improperly spliced. 29 CFR 1926.452(a)(9): Where wooden poles are spliced, the ends shall be squared and the upper section shall rest squarely on the lower section. Wood splice plates shall be provided on at least two adjacent sides, and shall extend at least 2 feet (0.6 m) on either side of the splice, overlap the abutted ends equally, and have at least the same cross-sectional areas as the pole. Splice plates of other materials of equivalent strength may be used.      a) Stairwell B:       Two employees constructed a wood pole scaffold on the staircase around the stairwell from basement level to roof bulkhead and worked from this scaffold. Where wooden poles are spliced on this scaffold, the upper sections were not rested on the lower sections, and wood splice plates were not provided on either side of the splices.
