This violation most recently occurred on or about April 2, 2013, at a workplace located at 729 Third Avenue, Dallas, TX 75226; where at least one (1) employee, who was repairing/replacing skylights, was not protected from falling through the skylights and was exposed to a fall hazard of approximately 25 feet to the lower level.
