29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary. On or about April 5, 2013, employees who were performing stucco work were exposed to a fall hazard. Wooden scaffold planks were not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide (fully decked). The fall distance to the ground where employees worked at different locations on the scaffold was approximately 12 feet.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees. On or about April 5, 2013, an employee stood on a Styrofoam block that had been placed on the deck of a working level that was built on side brackets / cantilevers above the main working level platform. The Styrofoam block was used to add height so that the upper area of the building wall was accessible during stucco work. This created a fall hazard should the block become unstable. The fall distance to the ground surface below from the Styrofoam block was approximately 17 feet.
