29 CFR 1910.269(c): The employee in charge did not conduct a job briefing with the employees involved before they started each job, including at least the following subjects: hazards associated with the job, work procedures involved, and special precautions: At the AMEREN-UE Calloway Nuclear Plant, switchyard, specifically at Bus B, 52-4, an arc flash occurred on or about 04/02/13 severely burning employees. The job briefing performed prior to the arc flash did not contain the following elements: a/ Ensuring that the conductors inside of Bus B, 52-4, were isolated from all sources of electrical energy prior to the start of work. b/ Identifying the minimum safe working conditions. c/ Identifying the type of personal protective equipment to be worn.

The employees, normally assigned to work with electrical equipment rated at 480 volts or less, were tasked with grounding Bus B, 52-4, rated at 13.8 kv.

Employee(s) approached to within six inches of the live connectors located inside of this piece of equipment. Bus B, 52-4 was powered by 13.8kv at the time. Table R-6 requires that a minimum approach distance of 26 inches be maintained for that amount of voltage.

The employees were not provided with or wore suitable personnel protective equipment such as 40-calorie suits or the equivalent prior to completely isolating this piece of equipment from its 13.8kv electrical source.
