Natural and synthetic fiber rope slings shall be immediately removed from service if any of the following conditions are present such as broken or cut fibers:        a) On or about April 5, 2013, in the Production Shop, the employees were using a 4 foot wide synthetic fiber sling that was severely cut and frayed, exposing employees to the hazard of being struck by the load.

Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel.       a) On or about April 5, 2013, in the production department, no tongue guards were provided on the 10" Delta Bench Grinder, exposing employees to the hazard of being struck by the wheel.
