(c) On or about 04/24/13 - Tool & Die Maintenance Shop, Sharp Milling Machine, the rotating chuck was not guarded.    (d) On or about 04/24/13 - Tool & Die Maintenance Shop, KMA-3 Sharp-KMA Milling and Drilling Machine, the rotating spindle was not guarded.      Matsu Alabama, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212 (a)(1), which was contained in OSHA inspection number 314838608, citation number 1, item number 3 and was affirmed as a final order on 08/08/2011, with respect to a workplace located 9650 Kellner Road SW, Huntsville, Alabama.
