29 CFR 1910.67(c)(2)(v): The employer did not ensure a body belt was worn and a lanyard attached to the boom when working from an aerial lift. An employee performing maintenance to lights in the parking lot was exposed to a fall hazard while operating a vehicle mounted lift and the employer did not ensure a personal fall arrest system or a fall restraint was utilized.
