29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net system, personal fall arrest system, or a monitor system.    a)  At the jobsite, on the roof, on or about 4/5/13:  Employees were engaging in roofing work at approximately 13 feet from ground without fall protection of any kind.

29 CFR 1926.150(a)(2): Access to all available firefighting equipment was not maintained at all times    a)  At the jobsite, on the roof, on or about 4/5/13:  Employees were engaging in roofing work at approximately 13 feet from ground and using various flamable adheasives and using compressed gas to "burn" seals in the roof membrane.
