29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  a) Caterpillar TH63 Telehandler - Employees were observed working from an elevated scaffold work platform and the platform was not securely attached to the Caterpillar TH63 telehandler exposing the employees to an approximate 20 foot fall hazard, on or about April 5, 2013.  b) Caterpillar TH63 Telehandler - An employee was observed standing on an elevated scaffold work platform placed on the forks of a Caterpillar TH63 telehandler while the machine was being moved in reverse exposing the employee to a fall hazard, on or about April 5, 2013.
