29 CFR 1926.452(w)(6)(i): The surface on which the scaffold was being moved was not free of pits, holes, and obstructions; (a) Orion Environmental, 10301 Washington St., Thornton, CO 80221; On and before April 10, 2013, the exposing employer did not ensure that the path over which a movable scaffold was to be moved was free of obstructions. Specifically, as the employer removed ceiling tiles and other items from the ceiling, he allowed them to accumulate on a floor on which there was a substantial amount of standing water and become clumps on which the scaffolds wheels became stuck, causing an employee to fall approximately 7 feet.
