29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) At the site, where an employee was working from a JLG aerial lift, model #450AJ SII, serial #0300073873, without wearing a body belt and lanyard. The employee had a rope tied around his waist while he was performing window washing activities from the aerial lift guard railing system. This condition exposed the employees to a fall hazard.
