29 CFR 1926.501(b)(4)(i):   Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  (a) Location - 475 Boston Road, Billerica, MA:   On or about 1/31/13 and 2/1/13, the employer failed to protect employees from the stairway hole opening in the second floor attic.  The workers were exposed to fall hazards of greater than 10-feet while working from the second floor.

29 CFR 1926.1052(c)(1)(ii):   Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge.  (a) Location - 475 Boston Road, Billerica, MA:   On or about 1/31/13 and 2/1/13, the employer failed to provide handrails on the stairway for the workers working on the second floor.  The workers were exposed to fall hazards of greater than 10-feet while accessing and exiting the stairway.
