29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a) The employer did not establish energy control procedures consisting of a written program and training procedures to address periodic reviews, outside contractor and employee training.    b) On or about February 1, 2013 the electrical disconnect on the AirLanco air filtering unit (baghouse) was not actuated to disable power while changing the filter bags.    c) On or about February 1, 2013 a lockout device(s) were not attached to the energy isolating point on the AirLanco air filtering unit(baghouse).  f) On or about April 10, 2013 the electrical disconnect on the Columbia robot(pallet machine) was not actuated to disable power while unjamming and removing/replacing material inside of the enclosed cage.    g) On or about April 10, 2013 a lockout device(s) were not attached to the energy isolating point on the Columbia robot(pallet machine).

29 CFR 1910.272(g)(1)(ii): All mechanical, electrical, hydraulic, and pneumatic equipment which presents a danger to employees inside grain storage structures shall be deenergized and shall be disconnected, locked-out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods.    d) On or about February 9, 2013 the electrical disconnect on the Bin Sweep was not actuated to disable power while removing grain material(s) from the bin.    e)    On or about February 9, 2013 a lockout device(s) were not attached to the energy isolating point on the Bin Sweep.

29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.   a) In the grain loading area markings were not in place in the aisles to protect electrical disconnect panels from contact with powered industrial lift truck.
