29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: Throughout the facility at the Jobsite: The employer is not evaluating and training each operator of powered industrial trucks within the time frames required by the standard. The employer is not providing formal evaluation nor practical training for all of the operators on site within three (3) years of their last evaluation and training. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
