29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (The requirements of 1910.1200 are applicable to construction work per 29 CFR 1926.59.) Farmland Foods, 200 So. 2nd St. Lincoln, NE - Work areas: Coolers 5 & 6 & Blast area #2 - Employee(s), exposed to carbon monoxide from gas/diesel equipment operating in an enclosed area and to silica from the removal of a cement containing wall/concrete floor, had not received the hazard training required by the standard. Employee(s) were unaware of the symptoms of carbon monoxide exposure and of the hazards/presence of silica in the wall/flooring.

29 CFR 1926.55(a): Employee(s) were exposed to material(s) at concentrations above those specified in the Threshold Limit Values of Airborne Contaminants for 1970 of the American Conference of Governmental Industrial Hygienists. Farmland Foods, 200 So. 2nd St. Lincoln, NE - April 6, 2013 - Work areas: Coolers 5 & 6 & Blast area #2 - Employee(s), engaged in concrete/debris removal, were exposed to levels of carbon monoxide exceeding the Permissible Exposure Limit of 50 parts per million (ppm). Exposed employee(s) had carboxyhemoglobin levels reaching 16%, as measured by the Lincoln Fire Department Medic Unit and Bryan Medical Center. Lincoln Fire Department monitoring also indicated that carbon monoxide levels exceeded 450 ppm in the work area.
