29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Rob Lebarr, Toledo, Ohio:  On or about April 4, 2013, at the address of 235 Sinclair St., Toledo, Ohio, the employer did not ensure that an employee was protected from falls while removing a portion of a brick parapet wall from a commercial structure.  The employee was exposed to a fall of approximately 14 feet to the ground below, which could result in permanent disability or death.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) Rob Lebarr, Toledo, Ohio:  On or about April 4, 2013, at the address of 235 Sinclair St., Toledo, Ohio, the employer did not ensure that employees had been trained in the recognition of fall hazards and how to minimize these hazards with the use of fall protection systems.  One employee was exposed to falls of approximately 14 feet to the ground below, which could result in permanent disability or death.
