Area wipe samples indicated that 5.7 milligrams of lead dust was present on the work table adjacent to the steam cabinet before lead-based paint removal. This condition exposed employees to lead health hazards. b) Garner-Lyons Historic Windows, LLC, dba: Lyons Historic Window @ 5360 Washington St., #C, Denver, CO 80216: On 6/26/13, the employer has not implemented methods or tools to ensure that surfaces were maintained as free as possible from accumulation of lead. Area wipe samples indicated that 0.88 micrograms of cadmium dust was present on the work table adjacent to the steam cabinet before lead-based paint removal. Employees dispensed Palco-2, a methylene chloride based solvent, from a five gallon carboy into a smaller pale. Employees then applied Palco-2 to the sashes with a four inch paint brush. This condition exposed employees to eye hazards related to corrosive chemical burns.

Employees were not made aware of the corrosive nature of methylene chloride and what to do in the event of eye contact with Palco-2, a methylene chloride based solvent. Employees were required to wear 3M 6200 Series, half-faced, air-purifying respirators equipped with combination filter in the wood restoration department during solvent application and paint scraping activities and in the painting department where water-borne paints were applied to restored and newly fabricated window sashes. b) Garner-Lyons Historic Windows, LLC, dba: Lyons Historic Window @ 5360 Washington St., #C, Denver, CO 80216: On and before 4/8/13, the employer did not establish and implement a respiratory protection program. Employees were allowed to wear 3M 8210 Series, N95 filtering face pieces in the wood restoration department during sash hardware removal and in the milling department for general dust comfort.

He was monitored for 101 minutes and was exposed to an 8-hour Time Weighted Average of 10.2 parts per million. His calculated severity was 0.41.
