29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:  a) On or about April 10, 2013, employees were exposed to a caught-in hazard with an unguarded rotating shaft measuring approximately 1 & 1/2 inches wide by 6 inches long attached to the Varidrive Module Electric Motor Model D04E740F.   b) On or about April 10, 2013, employees were exposed to a caught-in hazard with an unguarded rotating shaft measuring approximately 1 & 1/2 inches wide by 2 & 1/4 inches long attached to the Dayton Linear Actuator Model 4Z847.
