29 CFR 1926.62(i)(2)(ii): The employer shall assure that change areas are equipped with separate storage facilities for protective work clothing and equipment and for street clothes which prevent cross-contamination. a. On or about April 10, 2013, at the above addressed jobsite, an employee was conducting grinding operations on a structure coated with lead based paint, the employer did not ensure the conex box was equipped with separate storage facilities, thereby exposing the employee to lead hazards.

29 CFR 1926.502(d)(6)(i): Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged in accordance with 1926.502(d)(6)(i) through (6)(v): a. On or about April 10, 2013, at the above addressed jobsite, an employee conducting grinding operations on a bridge was improperly tied off to the wire rope of a retractable life line, thereby exposing the employee to fall hazards of approximately twenty (20) feet.
