On or about April 09, 2013 - at the above addressed jobsite, the foreman was exposed to fall hazards of approximately eight (8) feet three (3) inches due to the safety monitor having other responsibilities, such as assisting the foreman with his work activities.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. a. On or about April 09, 2013 - at the above addressed jobsite, employees were not provided with safe means of access when accessing the sloped roof from the flat roof, thereby exposing employees to a fall hazard.
