29 CFR 1910.305(b)(2)(i):       All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.        a) In the Main Production Building, northeast section, east wall, the 110 volt electrical receptacle was missing its faceplate, exposing the inner electrical wiring connections.

29 CFR 1910.334(a)(2)(i):       Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):       a)    In the Main Production Building, northeast section, east wall, the yellow 110 volt electrical extension cord receptacle supplying electricity various equipment , had damage/openings  in its outer insulation.  The outer insulation also was pulled away from plug, allowing strain to be placed the inner electrical connectors.        b)    Outside, north of the Production Building, on the fuel tanks, the orange 110 volt extension cord outer insulation  was pulled away from plug, allowing strain to be placed the inner electrical connectors.  It was spliced together with wing nuts.

29 CFR 1910.1200(e)(1):         Employer had not developed or implemented a written hazard communication program which at included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):                 a)    A written Hazard Communications program regarding hazardous chemicals had not been developed to inform and train employees to protect them from chemicals hazards.  Employees working with hazardous chemicals  such as but not limited to motor brake fluid, hydraulic oil and cement were exposed to the hazards associated with those products.
