29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about April 10, 2013, the company's large roll form machine was not guarded to protect employees from an ingoing nip point at the infeed of the machine. b. On or about April 10, 2013, guarding on the top of the company's large roll form machine left gaps measuring approximately 3 inches, which would not adequately prevent employees from reaching into the machine. General Awning Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 665938, citation number 1, item number 4a and was affirmed as a final order on November 26, 2012, with respect to a workplace located at 1350 E. Granger Rd., Brooklyn Heights, OH, 44131.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: a. On or about April 10, 2013, guarding used to protect employees from hazards associated with rotating parts on the company's U-channel machine was not affixed. b. On or about April 10, 2013, guarding present on the company's large roll form machine was secured with Velcro, requiring no special tooling for removal. General Awning Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(2), which was contained in OSHA inspection number 665938, citation number 1, item number 4b and was affirmed as a final order on November 26, 2012, with respect to a workplace located at 1350 E.
