On or about April 10, 2013, guarding on the top of the company's large roll form machine left gaps measuring approximately 3 inches, which would not adequately prevent employees from reaching into the machine.    General Awning Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 665938, citation number 1, item number 4a and was affirmed as a final order on November 26, 2012, with respect to a workplace located at 1350 E. Granger Rd., Brooklyn Heights, OH, 44131.
