29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a. On Area D roof where metal studs were being cut by a DeWalt Chop Saw and then installed as wall joists, the extension cord used to supply power to the Chop saw was pinched between the concrete and the bottom of the metal parapet wall thereby exposing the employees to a contact hazard.

29 CFR 1926.502(b)(3): Guardrail systems shall be capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge. a. On the second level of the building where employees were working with sheet metal to install curbs, Barton Marlow installed a red danger tape as a guardrail system to prevent falls off the edge of the floor thereby exposing employees to a fall hazard.
