29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:     (a) Jobsite, Theater Drive, Duncansville, PA:  On or about 04/09/13 an employee was working at the edge of the roof operating a crimper without any fall protection systems in place.  This condition exposed the employee to fall hazards of approximately 27 feet to the ground below.

29 CFR 1926.761(b)(2): The employer did not provide a training program that included the use and operation of guardrail systems (including perimeter safety cable systems), personal fall arrest systems, positioning device systems, fall restraint systems, safety net systems, and other protection to be used:      (a) Jobsite, Theater Drive, Duncansville, PA:  On or about 04/09/13 an employee was operating a crimper at the edge of the roof area without utilizing fall protection exposing the employee to a 27 foot fall hazard.  The employee had not been trained to recognize and understand when fall protection systems were necessary to protect themselves' when performing steel erection activities.
