29 CFR 1926.651(c)(2): Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that was 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. A. Latcha Inc. DBA D & K Excavating worksite located at 6922 Spring Valley Dr., Holland, Ohio: On or about April 9, 2013, the employer did not ensure a safe means of egress such as a ladder was provided to employees installing a waterline in a trench which was approximately 6.60 feet deep with near vertical walls composed of "C" soil. Latcha Inc. DBA D & K Excavating was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.651, which was contained in OSHA inspection number 311607881, citation number 1, item number 1 and was affirmed as a final order on April 17, 2009 with respect to a workplace located at Oregon, Ohio.

DBA D & K Excavating worksite located at 6922 Spring Valley Dr., Holland, Ohio: On or about April 9, 2013, the employer did not ensure employees were adequately protected from cave-in/collapse hazards while installing a waterline in a trench which was approximately 6.60 feet deep with near vertical walls composed of "C" soil. Latcha Inc. DBA D & K Excavating was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652, which was contained in OSHA inspection number 311607881, citation number 1, item number 2 and was affirmed as a final order on April 17, 2009 with respect to a workplace located at Oregon, Ohio.
