29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 10, 2013, at 1605 Creek Bend Lane, Lawrenceville, Georgia:  An employee performing framing work was exposed to struck-by hazards from falling nail guns, saws, and plywood from the employees working above him when he did not use a hard hat.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about April 10, 2013, at 1605 Creek Bend Lane, Lawrenceville, Georgia:  a) Four employees performing framing work were exposed to struck-by hazards from nails and wood chips when they used Hitachi NR83A2(S1) nail guns without using eye protection.   b) Four employee performing framing work were exposed to struck-by hazards from wood chips and saw dust when they used Makita 5007F radial saws without using fall protection.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage:  On or about April 10, 2013, at 1605 Creek Bend Lane, Lawrenceville, Georgia:  Four employees performing framing work were exposed to electrical shock hazards while operating Makita 5007F, 120 volt, 15 amp, radial saws connected to flexible cords with cuts, tears, missing sheathing and strain relief damage, when the employer did not remove the cords from service.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about April 10, 2013, at 1605 Creek Bend Lane, Lawrenceville, Georgia:  a) Two employees performing framing work were exposed to a fall hazard of approximately 10.8-feet when they worked at the unprotected edge of the building without using fall protection.   b) Four employees performing framing work were exposed to a fall hazard of approximately 10.2 to 10.8-feet when the employer did not protect the employees from falling through the two unprotected floor openings for the stairwells.
