29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about April 8, 2013, at the worksite located at 300 North Spruce in Searcy, Arkansas, east side of roof, employee on the approximately 6 ft deep outside window ledge was not protected from falling to lower levels. Employee was exposed to falls from elevation.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: On or about April 8, 2013, at the worksite located at 300 North Spruce in Searcy, Arkansas, employees working on the north side of an approximately 5:12 pitched roof performing roof renovation tasks such as but not limited to removing clay tile, were not protected from falling to lower level. Employees were exposed to falls from elevation.
