Employees were exposed to an approximately 14 ft. fall from elevation.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about April 8, 2013, at the worksite located at the Corner of South Spring & Mulberry Street in Searcy, Arkansas, the Werner extension ladder used to access and descend from the roof was not secured and extended approximately 14" above the roof landing surface.  This condition exposed employees to the hazard of falls from elevations.    Scott A. Michaels was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 315174037, citation number 1, item number 3 and was affirmed as a final order on March 28, 2011 with a final abatement date of March 10, 2011, with respect to a workplace located at 515 South Main Street in Searcy, Arkansas.
