29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) On or about April 10, 2013 the inspection revealed that employees were potentially exposed to serious injury from working in close proximity to an electric circuit that was not effectively guarded. The hazard was located in the electrical/mechanical room of building 255 at the Portland Air National Guard Base in Oregon.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: (a) On or about April 10, 2013 the inspection revealed a flexible cord with damaged outer insulation on the Dewalt angle grinder D28402, serial number 837157 with the wire brush attachment. The tool was being used outside adjacent to building 255 at the Portland Air National Guard Base in Oregon.
