OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the conditions listed below. Stairway leading up to the shale shaker area from ground level; On or about March 5, 2013 the employer did not ensure that a stairway used by employees to access to and from the rig floor was secured from being displaced/dislocated. Pins or other suitable methods were not employed to secure the stairway. This condition exposed employee(s) to 10 feet fall hazards.
