Location: 83 West 118th Street, New York, NY 10026, front exterior of the building.  On or about 04/10/2013  a). Two Employees were working beneath a two point light weight scaffold system in front of the building, without wearing hard hats thus exposing themselves to injury from overhead falling object hazards.

29 CFR 1926.451(d)(3)(x): Tiebacks shall be installed perpendicular to the face of the building or structure, or opposing angle tiebacks shall be installed. Single tiebacks installed at an angle are prohibited.  Location: 83 West 118th Street, New York, NY 10026, Roof top location;  On or about 04/10/2013  a). Employees were observed working on a two point, light weight, rope suspension scaffold system with tie backs.  The tie backs were not secured to a fixed anchor point that was installed perpendicular (at right angle) to the face of the building.

29 CFR 1926.451(d)(5)(ii): Suspension scaffold supports devices, such as cornice hooks, roof irons, parapet clamps or similiar devies were no supported by bearing blocks.  Location; 83West 118th Street, New York, NY 10026; front cornice of building.  On or About 04/10/2013;  a). Employees were observed working on a light weight, two point rope suspension system. The Cornice hooks (C-Hooks) did not have any support blocking between the cornice and the hook creating a three point contact point as required.

Location; 83 West 118th Street, New York, NY 10026, front exterior of the building.  On or About 04/10/2013;  a). Employees were working on a two point light weight rope suspension scaffold, the employees did not attach/affix/secure the lifelines to an independent point of anchorage from the scaffold system.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Location: 83 West 118th Street, New York, NY 10026  On or About April 10, 2013  a). Employees were working on a sidewalk shed roof top and working platform with an exposed edge. Employees were working close to the edge and were exposed to a potential fall of 8 feet to the roof top below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.   Location 83 West 188th Street, new York, NY 10026, front exterior of building.   On or about 04/10/2013;   a). Employees were using portable ladders to gain access to the top of the sidewalk shed and working platform in front of the building, but did not have the ladder side rails extending 3 feet above the upper landing surface.
