29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) North Preston Street side of building - The working levels of a tubular welded frame scaffold were not fully planked or decked while employees were stripping and repointing brick face exposing employees to a fall hazard of approximately twenty-eight (28) feet.  Observed on or about April 8, 2013.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) North Preston Street side of building - Employees stripping and pointing brick on a tubular welded frame scaffold were exposed to a fall hazard ranging from fourteen (14) to twenty-eight (28) feet without the use of a personal fall arrest system or guardrail systems.  Employees were working on the second and fourth levels of the scaffold.  Observed on or about April 8, 2013.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:   a) North Preston Street side of building - Employees were not provided with a safe means of access and/or egress to the tubular welded frame scaffold platform, exposing employees to a fall hazard of twenty-eight (28) feet above the ground level.  Employees were using the side frame of the scaffold to access and/or egress from the working levels.  Observed on or about April 8, 2013.
