29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a. Production area/hydraulic punch press # 24-A - the employer's did not develop, document and utilize an energy control procedure (lockout/tagout program) and did not provide training on the energy control procedure to ensure that all required elements of the procedure were understood by the employees and that the kwnoledge and skills required for the safe application, usage and removal of the energy controls were acquired by employees performing servicing or maintenance on a machine or equipment such as the - Cincinnati  press brake (Serial # 52492).

29 CFR 1910.133(a)(3): The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.    a. Production Area - On or about April 10, 2013, the employer had not ensure that employees who needed prescription lenses while performing maintenance operations on press brakes (hydraulic/Cincinnati # 23A with serial # 52492) wore eye protection. Employees were thereby exposed to eye hazards.

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.   a. Production Area/hydraulic power press/press brakes - On or about April 10, 2013, the Cincinnati brake press (Hydraulic/Serial # 52492) was inadequately guarded to prevent other employees from reaching into the point of operation. Employees were thereby exposed to the hazards of moving equipment.
