Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards and thermal burns:    (a) On or about April 11, 2013, for employees exposed to an approximate 60 foot fall hazard and who were not provided a ready means of escape from the derrickman's platform in the event of a well blowout or other emergency.  The escape line had not been secured and/or anchored on Rig 718, located at Satter 44-34-SWH, near Watford City, North Dakota.    Abatement Note: Among other methods, one feasible and acceptable method of abatement would be for the employer to install the emergency descent device in accordance with the Rollgliss Rescue Emergency Descent Device User Instruction Manual, pages 2-6.  Also, (API) American Petroleum Institute Recommended Practice 54, Section 6.10.1 which includes, among other requirements, the following: "On all land rigs, the derrick or mast shall have an auxiliary means of escape installed prior to personnel working in the derrick. The auxiliary escape route should use a specially rigged and securely anchored escape line attached to the derrick or mast so as to provide a ready and convenient means of escape from the derrickman's working platform. The escape line route should be kept clear of obstructions."
