Authorized employees performing the work were not locked out nor were they provided information and training on the purpose and function of energy control practices.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures (a) While at the 1625 Leider Drive, Union Grove, Wisconsin jobsite, the employer did not share their lockout program with Coleman Tool & Mfg. Co. Employees were tasked to clean a robotic welder after fire damage and did not comply with either the onsite employer's lockout/tagout program nor their own.

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area and chemical-specific information was not always available through labels and material safety data sheets. (a) The employer did not ensure that information and training was effectively communicated to employees working with chemicals, such as but not limited to Genuine Unsmoke Degrease All and Odor X 9-D-9. Employees were performing cleanup and restoration activities at the 1625 Leider Drive, Union Grove, Wisconsin temporary worksite without adequate training.
