G.E.M. Building Corporation, Little Creek Base, Navy Lodge: On or about 04/16/13 employees on the second floor installing floor planks were using a vertical lifeline where the snaphook was wrapped around a steel beam and connected to itself, creating a fall hazard of 11 feet 4 inches.
