29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a. On or about April 8, 2013, four employees removing and replacing shingles on the 5:12 pitch roof of Apartment Building A (Apartments 049-056) were not protected from a 15-foot fall hazard by a guardrail sytem, safety system, or personal fall arrest system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at its top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder:     a. On or about April 8, 2013, at Apartment Building A, employees used a portable extension ladder which did not extend at least 3-foot above the landing surface, exposing themselves to a 15-foot fall hazard.
