29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  Employees operating drill presses were exposed to struck-by hazards. The employees were exposed in that safety shields were not on the drill presses identified as:  a) Skill Model # 3320, in the Prescription Room. b) Powermatic Model # 1150, Serial # 815V1145, in the Production Room. c) Bridgeport Model #F537, Serial #1150001, in the Maintenance Room.

29 CFR 1910.178(a)(6): The user shall see that all nameplates and markings are in place and are maintained in a legible condition.  On site at the facility the side activation control lever for the forklift (Yale forklift) was not identifiable with markings to indicate the actions of the lever.

29 CFR 1910.303(g)(1)(i)(B): The width of working space in front of the electric equipment shall be the width of the equipment or 762 mm (30 in.), whichever is greater. In all cases, the working space shall permit at least a 90-degree opening of equipment doors or hinged panels.  In the Production Room, access to an electrical panel box was blocked by portable table with a machine identified as ACE.

29 CFR 1910.305(a)(2)(ix): All lamps for general illumination shall be protected from accidental contact or breakage by a suitable fixture or lampholder with a guard. Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded.   In the Prescription room, the lamps in use were not protected from accidental breakage by an employee.
