The 4-foot Verson mechanical press brake, Model 930, Serial No. 21042-1648.  On or about April 10, 2013, and at times prior thereto, employees were exposed to an amputation hazard from the back side of the press being unguarded.

29 CFR 1910.212(a)(1):   One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Near the east end of the Production Room.  Pratt and Whitney jigborer, Model 2A.  On or about April 10, 2013, and at times prior thereto, employees were exposed to an amputation from the lack of a protective device near the point of operation while milling metal tools.   b)  Near the north wall of the Production Room.  Enco drill press, Model 40030.  On or about April 10, 2013, and at times prior thereto, employees were exposed to an amputation from the lack of a protective device near the point of operation while drilling metal tools.  c)  Near the east end of the Production Room.  DoAll bandsaw, GE Model 5KH45AB55B.  On or about April 10, 2013, and at times prior thereto, employees were exposed to an amputation from the lack of a protective device near the point of operation while sawing metal tools.

On or about April 10, 2013, and at times prior thereto, employees were exposed to an amputation hazard, from the 10-foot Wisconsin Forcemaster pneumatic press brake, Model 10FM150, Serial No. 73370 not being guarded at the point of operation, while they held the sheetmetal approximately 4 inches from the stamper and dye.  b)  Near the south wall of the Production Room.  On or about April 10, 2013, and at times prior thereto, employees were exposed to an amputation  hazard, from the 6-foot Niagara mechanical press brake, Model 1B36-5-6, Serial no. 46855 not being guarded at the point of operation, while they held the sheetmetal approximately 4 inches from the stamper and dye.   c)  Near the south wall of the Production Room.  On or about April 10, 2013, and at times prior thereto, employees were exposed to an amputation hazard, from the 4-foot Verson mechanical press brake, Model 930, serial no. 21042-1648 not being guarded at the point of operation, while they held the sheetmetal approximately 4 inches from the stamper and dye.
