Integral prefabricated scaffold access or direct access from another scaffold, structure, personal hoist, or similar surface was not used: A) 2754 Hylan Blvd, Staten Island, NY 10306: Employees were exposed to fall hazards of approximately 14' 8" to the ground below when a ladder was not provided for safe access to a tubular welded scaffold platform.
