On or about April 12, 2013.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:      1) Location:  Reserve at Becks Pond, fourth-floor elevator landing - Employees were exposed to a fall, approximately 40 feet to the bottom of the elevator shaft, as they stood approximately two feet atop scaffolding that did not have guardrails and was positioned on the landing along the width of the open elevator shaft.  Employees were standing on the scaffolding, reaching into the elevator shaft during construction of the elevator shaft opening top lintel.  On or about April 12, 2013.      REPEAT VIOLATION:  Ashcraft Masonry, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.451(g)(4)(i), which was contained in OSHA inspection number 311805733, citation number 1, item number 1, and was affirmed as a final order on (date), with respect to a workplace located at 3216 Brookline, Wilmington, DE 19810.
