On or about November 15, 2012, and times prior thereto, employees were not wearing eye protection while performing activities that included but were not limited to, utilizing a metal hammer to strike metal hammer unions. Condition potentially exposed personnel to eye hazards from flying metal particles.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a) Well site-M4, rig platform and surrounding areas. On or about November 15, 2012, and times prior thereto, employees working underneath the raised mast of the service rig during fracking operations were not wearing protective helmets/hard hats when potentially exposed to falling objects.
