Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about 4/11/2013, employees were not protected from falling while working from an elevated work surface greater than six feet above a lower level.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. (a) Location - 175 Fellsway West, Somerville, Ma: On or about 4/11/2013, the employer did not provide adequate training for the employees who might be exposed to fall hazards.
