29 CFR 1910.145(c)(3):     Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s):    On or about April 1, 2013 at Helmerich & Payne Rig 399, employees were exposed to potential struck-by hazards during high-pressure stump testing of the blowout preventer.  The buffer zone marked by plastic caution/danger tape was only partially established and marked, leaving the backside of the BOP transport skid near the stair tower from the rig floor and on the end of the transport skid nearest the rig motors unmarked.    Among other methods, one feasible means of abatement would be to add purpose-built signs reading DANGER:HIGH PRESSURE TESTING to the plastic barrier tape at each high traffic area leading into the buffer zone.
