29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On March 11, 2013 at a worksite located at 263 Dupage Street Elgin IL, an employee was working from a roof removing gutters. The employee was not utilizing any means of fall protection. The employee was exposed to a fall hazard of approximately 12 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903(d).

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On March 11, 2013 at a worksite located at 263 Dupage Street Elgin IL an employee was accessing the roof by an extension ladder that was not three feet above the landing. The employee was exposed to fall hazards while accessing the roof. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903(d).
