FTA-2, a Spuhl coil assembly machine, serial number 408858:  The coil former section of the machine was not provided with an interlocked door and the in-running nip points created where the vertical assembly belts ran outside of the machine on the side of the assembly table.   E.  FTA-3, a Spuhl coil assembly machine, serial number 408700:  The coil former doors were not provided with interlocks and an employee was observed inside of the enclosure making adjustments to the machine as it ran at production speed.
