29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs: Frame Bending Area: On, or about, April 12, 2013, the fixed stairs providing access to the rear platform of the Innovatech Frame Bender Machine did not offer a uniform riser height in that the top step was installed too low as compared to the platform height and a large step was required for the operator to step off the top step onto the platform.

CS-1, a Spuhl Model CS-420-Z coil assembly machine, serial number 415278: The interlocks provided on the coil former access doors, transfer section access doors and assembly table access door were bypassed. B. CS-2, a Spuhl Model CS-420-Z coil assembly machine, serial number 415212: The interlock provided on the coil former access door was not operable and a guard was missing from the assembly table access door that left a 9.5 inch by 12 inch opening. C. FTA-1, a Spuhl coil assembly machine serial number 408696: Operable interlocks were not provided on the coil former section or at the coil assembly area. Also, in-running nip points were accessible where vertical belts ran outside of the machine on the side of the assembly table. D. FTA-2, a Spuhl coil assembly machine, serial number 408858: The coil former section of the machine was not provided with an interlocked door and the in-running nip points created where the vertical assembly belts ran outside of the machine on the side of the assembly table. E. FTA-3, a Spuhl coil assembly machine, serial number 408700: The coil former doors were not provided with interlocks and an employee was observed inside of the enclosure making adjustments to the machine as it ran at production speed.
