29 CFR 1926.1052(c)(4): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not provided between the top rail of the stairrail system and the stairway steps: (a) West stair system to 2nd level: On or about April 10, 2013, and at times prior, workers were exposed to falling up to 77.75 inches to the next lower level from each of three stair courses.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems which met the criteria contained in Subpart M of 29 CFR 1926: (b) West stair system, second landing between first and second floor: On or about April 11, 2013 and at times prior, workers were exposed to falling approximately 155 inches (12 feet 11 inches) from the first stairwell landing where a guardrail system was installed with no midrail or equivalent member.
