29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: On or about 4/9/13 Location: 115 Fairview Park Drive, Elmsford, NY 10523 a) A 100 ft. extension cord used to power a Mikita right angel grinder ran from an interor wall outlet to the exterior work area. The cord ran through two metal clad doors which were closed upon it. The cord was not protected from damage.
