29 CFR 1910.23(d)(1)(iii): Flight(s) of stairs with 4 or more risers, less than 44 inches wide and having both sides open were not equipped with one standard stair railing on each side: (a) Bonanza Creek Energy, Inc. at Pronghorn TP4-HNB, Briggsdale, CO 80611: On and before April 11, 2013 the controlling employer did not ensure that the flight of stairs attached to the side of the pipe ramp on the drilling rig, leading from the ground level to the rig floor, was equipped with a proper standard handrail on the ramp side in that the stairs were 28 inches wide with approximately 14 risers. Subcontractor employees assessed the stairway. This condition exposed subcontractor employees to a fall hazard of approximately 15 feet down the pipe ramp.
